Provide screening, intake, assessment, individual and group counseling, case management, crisis intervention, client education and referrals in a 27-bed residential detox program. Maintain client records, assist with direct care tasks, consult with medical staff, and support treatment planning.

Provide HERO program case management: assess clients, develop and implement recovery/housing plans, document progress, link clients to housing, benefits, employment and community resources, perform outreach with landlords, complete required assessments and surveys, and participate in meetings and supervision.

Provide residential case management for adults with serious mental illness and/or substance use disorders: coordinate admissions, transportation, housing, discharge planning, assessments, advocacy, documentation, crisis intervention, group facilitation, and community resource linkage to support community reintegration.

The Clinician conducts assessments, provides counseling for substance abuse and mental health issues, develops treatment plans, and maintains client records.

The Case Manager will provide quality care, develop client-centered care plans, link clients to resources, facilitate life skill groups, and monitor clients' progress in various areas including mental health and substance abuse.
