Oversee kitchen operations, menu development, nutrition assessments, and wellness programs for memory-care and behavioral-health residents. Ensure regulatory compliance, staff training, inventory control, and family engagement to promote safe, appealing, and therapeutic dining experiences tailored to cognitive and behavioral needs.

Lead day-to-day operations of a Medicare-certified home health agency, ensuring regulatory compliance, high-quality patient care, operational efficiency, financial performance, staff development, and strategic growth. Oversee clinical and administrative teams, manage budgets, prepare for surveys/audits, drive quality improvement initiatives, and build community/referral relationships.

Provide psychosocial case management for clients with mental health and substance abuse needs: assess needs, create and monitor Individual Recovery Plans, coordinate services, maintain records, perform outreach, provide psychoeducation, and support daily living and treatment adherence.

Provide psychiatric assessment, diagnosis, medication management, lab ordering/interpretation, patient education, counseling, and referrals; collaborate with psychiatrists, therapists, and staff and perform follow-up care in an outpatient setting.

Provide in-home support to help clients live independently: run errands, assist with mobility, and counsel on independent living. Cover on‑call shifts Monday–Thursday as needed. Administer medications safely and document administration, monitor and report resident conditions, and support residents with dementia and behavioral health needs. Use dementia-friendly communication and behavior-management strategies, assist with ADLs, maintain safety/compliance, collaborate with nursing and care teams, and interact compassionately with families.

Build and maintain relationships with physicians, case managers, discharge planners, SNFs, hospitals, and community organizations to drive referrals and census growth. Perform regular outreach (calls and visits), conduct client/family intake, document outreach and referrals, represent the organization at events, collaborate with internal teams, and gather feedback to improve service delivery.

Provide person-centered nursing for residents with dementia and behavioral needs: administer medications, monitor and document physical/cognitive/behavioral changes, implement behavior-support and de-escalation strategies, assist with ADLs, follow safety and infection-control protocols, collaborate with interdisciplinary teams, and educate families.

Provide supervision and guidance to clinical staff to ensure quality care and documentation. Conduct monthly supervision, review and sign clinical documentation and treatment plans, monitor caseloads and QA reports, manage client grievances, document consultations in the EMR, develop treatment plans, and participate in crisis intervention as needed.
