Coordinate comprehensive mental health care by developing and monitoring treatment plans, collaborating with interdisciplinary teams, documenting in EHRs, advocating for clients, and providing virtual and in-person support to ensure continuity of care.

Administer medications safely and document administration, monitor and report resident conditions, and support residents with dementia and behavioral health needs. Use dementia-friendly communication and behavior-management strategies, assist with ADLs, maintain safety/compliance, collaborate with nursing and care teams, and interact compassionately with families.

Build and maintain relationships with physicians, case managers, discharge planners, SNFs, hospitals, and community organizations to drive referrals and census growth. Perform regular outreach (calls and visits), conduct client/family intake, document outreach and referrals, represent the organization at events, collaborate with internal teams, and gather feedback to improve service delivery.
