Provides patient-centered clinic care, including assessments, treatments, medication administration, care coordination, health education, documentation, follow-up, and quality improvement. Collaborates with providers and healthcare staff to support patient outcomes and delivers culturally competent care. Requires an active LPN license, nursing education, and driver’s license. Primary care and nursing experience are preferred.

Connect patients with community resources and support services through referrals, care coordination, social determinants of health screenings, benefits applications, and advocacy. Maintain accurate electronic patient records, track service outcomes, educate patients about eligibility and insurance options, and participate in community outreach. Collaborate with healthcare providers and social service agencies while helping vulnerable community members overcome barriers to care.

Provides patient-centered clinical care in a primary care setting, including patient assessments, treatments, medication administration, care coordination, health education, documentation, follow-up, and quality improvement. Collaborates with providers and healthcare staff while delivering culturally competent care to diverse patient populations. The full-time role offers a weekday schedule without nights or weekends.

Provides prescription verification, dispensing, patient counseling, medication inventory and formulary management, ordering, controlled-substance recordkeeping, regulatory compliance, quality assurance, and clinical team collaboration. The pharmacist also supports staff education, patient care standards, and pharmacy coverage across multiple Montana clinic locations when needed.

Provide patient-centered nursing care in a rural clinic: assess, treat, medicate, document, educate patients and families, collaborate with care teams, drive quality improvement, and engage the community.

Per diem pharmacist provides prescription verification, dispensing, patient counseling, medication inventory and controlled substance management, supports tele-pharmacy, collaborates with clinical teams on drug information and quality assurance, and models professional standards and customer service for rural clinic patients.
