Teams at One Health (formerly BVHC)

Provides patient-centered clinic care, including assessments, treatments, medication administration, care coordination, health education, documentation, follow-up, and quality improvement. Collaborates with providers and healthcare staff to support patient outcomes and delivers culturally competent care. Requires an active LPN license, nursing education, and driver’s license. Primary care and nursing experience are preferred.

Connect patients with community resources and support services through referrals, care coordination, social determinants of health screenings, benefits applications, and advocacy. Maintain accurate electronic patient records, track service outcomes, educate patients about eligibility and insurance options, and participate in community outreach. Collaborate with healthcare providers and social service agencies while helping vulnerable community members overcome barriers to care.

Provides patient-centered clinical care in a primary care setting, including patient assessments, treatments, medication administration, care coordination, health education, documentation, follow-up, and quality improvement. Collaborates with providers and healthcare staff while delivering culturally competent care to diverse patient populations. The full-time role offers a weekday schedule without nights or weekends.
