Analyzes preventive screening data, supports patient-centered medical home initiatives, participates in quality improvement and continuing education, and coordinates plans of care to improve patient outcomes.

Leads day-to-day operations across a community health center, overseeing workforce planning, budgets, facilities, quality improvement, compliance, provider productivity, grants, capital projects, and strategic initiatives. Partners with executives, board members, providers, staff, and community organizations to improve operational efficiency, patient outcomes, population health, and organizational sustainability.

Provides nursing care coordination, case management, triage, patient education, and preventative screening support for medical and behavioral health patients. Maintains timely EMR documentation, billing, and nurse visit scheduling. Develops coordinated care plans, monitors follow-up, supports Patient Centered Medical Home and Coleman initiative processes, contributes to quality improvement, and collaborates with providers and care teams.

Provide prescription management support under direction of licensed clinicians, participate in QI and training, comply with regulatory and facility policies, engage in patient-centered care teams, and perform other duties as assigned. Role may become hybrid after training.

Enter and verify patient charges and insurance data in the practice management system, confirm eligibilities, reconcile receipts and aging/credit reports, process and follow up on claims, provide estimates/predeterminations, support audits, and cross-train in dental billing and payment posting.

Provide HR support across recruiting, onboarding, benefits and payroll administration, maintain HRIS and employee records, manage health and loan/HRSA programs, ensure compliance and confidentiality, respond to staff questions, and support organizational initiatives.

Enter and verify patient and insurance billing data using the practice management system, confirm eligibilities, match charges to documentation and coding guidelines (CPT/CDT), manage reports (open encounters, aging, credit balance), post payments and EOBs, follow up on denied/delinquent claims via insurer portals, assist with Medicaid forms and audits, and provide cross-training and backup for related billing functions.

Provide clinical and clerical support in a school-based health center: room patients, record vitals, document in EMR, perform phlebotomy and CLIA lab controls, prepare treatments/procedures (non-medication), maintain supplies/cleanliness, assist with audits and scheduling, and participate in Patient-Centered Medical Home team initiatives.

Provide comprehensive primary care to patients of all ages, including preventive, acute, and chronic disease management. Diagnose, treat, perform exams, order and interpret tests, prescribe therapies, document in the EHR, coordinate care with behavioral health and specialists, participate in quality improvement, and support compliance with FQHC regulatory requirements.
