Teams at Northern Oswego County Health Services, Inc.

Provides nursing care, triage, care coordination, and case management for medical and behavioral health patients. Maintains timely electronic medical record documentation, billing, scheduling, and follow-up. Analyzes preventive screening data, supports patient-centered medical home initiatives, participates in quality improvement and continuing education, and coordinates plans of care to improve patient outcomes.

Leads day-to-day operations across a community health center, overseeing workforce planning, budgets, facilities, quality improvement, compliance, provider productivity, grants, capital projects, and strategic initiatives. Partners with executives, board members, providers, staff, and community organizations to improve operational efficiency, patient outcomes, population health, and organizational sustainability.

Provides nursing care coordination, case management, triage, patient education, and preventative screening support for medical and behavioral health patients. Maintains timely EMR documentation, billing, and nurse visit scheduling. Develops coordinated care plans, monitors follow-up, supports Patient Centered Medical Home and Coleman initiative processes, contributes to quality improvement, and collaborates with providers and care teams.
