Provides community-based behavioral health services for adults, including assessments, recovery planning, crisis intervention, psychosocial rehabilitation, resource navigation, referrals, and supported housing or employment assistance. Maintains continuity of care, coordinates with community partners, documents services in the electronic health record, and promotes independent living through trauma-informed, person-centered support.

Provides outpatient behavioral health and supported employment services across the LifePath system. Maintains a caseload, coordinates care, connects individuals with employment and community resources, delivers skills and symptom management support, documents services in the electronic health record, and assesses and de-escalates behavioral health crises. Frequently travels to homes, community settings, nursing facilities, and hospitals within Collin County while collaborating with internal teams and community partners.

Enrolls individuals with intellectual and developmental disabilities into Medicaid Waiver programs, assesses needs, develops service plans, coordinates referrals, and connects individuals and families with medical, social, vocational, financial, and community resources. Maintains accurate documentation, communicates with agencies and regulators, supports crisis de-escalation, addresses complaints, and provides culturally appropriate, trauma-informed, person-centered services.

Provides community, home, and school-based behavioral health services, including clinical eligibility assessments, applications, psychotherapy, treatment planning, crisis intervention, service coordination, documentation, and trauma-informed care. Requires independent clinical judgment, collaboration, timely records, and compliance with HHSC and related guidelines.

Reviews, investigates, verifies, processes, and resolves claims while ensuring accuracy, policy compliance, and adherence to regulatory requirements. Gathers supporting documentation, identifies discrepancies or potential fraud, escalates issues, and manages confidential information. The role also requires strong communication, organization, computer skills, and the ability to present information and instruct workforce members on applicable software.

Provides community-based behavioral health case management for adults on an ACT team, including assessments, recovery planning, referrals, supported housing and employment, skills and medication training, crisis intervention, psychosocial rehabilitation, and service coordination. Maintains documentation, participates in daily team huddles and on-call crisis coverage, supports access to healthcare and benefits, coordinates with community agencies, and promotes trauma-informed, person-centered recovery and independence.

Provides trauma-informed case management, skills training, medication support, and psychosocial rehabilitation to justice-involved individuals with serious mental illness or substance use disorders. Coordinates medical, social, vocational, financial, and educational services; completes assessments and service plans; documents care; manages scheduling and crisis procedures; and collaborates with criminal justice agencies to reduce recidivism and support community stability.

Provide financial eligibility support for behavioral health clients: complete initial and annual financial assessments, enter data, run reports, communicate with contracted providers and treatment teams, ensure compliance with TAC/UM/Medicaid, and deliver excellent customer service within a trauma-informed, person-centered model.

Provide face-to-face crisis assessments, intervention, relapse prevention, and 90-day follow-up for individuals with chronic mental illness in Collin County. Deliver community-based services at residences, schools, and other safe locations 24/7. Coordinate care, develop recovery plans, advocate for clients, provide routine case management, skills training, and psychosocial rehabilitation while maintaining clear progress notes and connecting clients to community resources.

Provide field-based mental health nursing and psychosocial rehabilitation within an ACT team: administer long-acting injectables, deliver case management, skills and medication training, crisis intervention, connect clients to services, document in the EHR, attend daily team huddles and participate in on-call 24/7 crisis rotation.

Provide 24/7 face-to-face crisis assessment, intervention, follow-up and relapse prevention for children, adolescents and adults in Collin County. Coordinate services, create recovery plans, document progress, connect clients with community resources, and provide routine case management and psychosocial rehabilitation over a 90-day period.
