Provide face-to-face crisis assessments, intervention, follow-up and relapse prevention for individuals with mental illness in Collin County. Deliver recovery planning, advocacy, case management, skills training, psychosocial rehabilitation and community resource education, working 24/7 across residences, schools and community locations. Coordinate services, document care, and follow clients for a 90-day period within federal, state and local guidelines.

Provide face-to-face crisis assessments, intervention, follow-up, intake and recovery/safety planning, resource navigation, referrals, and connections to outpatient behavioral health services at a diversion center, using trauma-informed, person-centered care.

Provide job coaching, skills training, job development, placement, and on-the-job support for individuals with intellectual/developmental disabilities. Conduct assessments, create individualized plans, build employer/community relationships, document progress in electronic records, and assist with life skills. Work includes travel in the community, variable hours, crisis de-escalation, and collaborative team staffing.

Provide community-based ACT case management: assessments, recovery planning, resource navigation, referrals, supported housing/employment, crisis intervention, and linkage to medical and social services. Work collaboratively with ACT team, community partners, and fidelity managers; document services in electronic health record; respond on call as needed; promote trauma-informed, strength-based, person-centered care to reduce hospitalizations and support independent living.

Provide clinical and administrative supervision for 1-2 MST teams, ensuring fidelity to MST principles, clinician competency, and 24/7 clinical support. Deliver individual and group supervision, manage referrals and caseloads, collaborate with stakeholders and MST experts, track clinical outcomes, and occasionally provide direct MST services to 1-2 families.

Designs, delivers, and tracks clinical onboarding and ongoing training for Substance Use Disorder programs. Serves as SUD subject-matter expert, teaches curricula (in-person and e-learning), maintains training records and CCBHC measures, cross-trains across programs, and supports workforce engagement, compliance, and quality standards.

Manage utilization review and quality management for behavioral health services. Review authorizations (prospective, concurrent, retrospective), ensure UM guideline adherence, monitor utilization patterns, coordinate with providers and MCOs, participate in audits and HHSC calls, and recommend process improvements. Does not provide direct clinical services.

Coordinate and deliver onboarding and ongoing clinical training for IDD programs. Serve as subject-matter expert on IDD care, trauma-informed and CLAS practices, develop curricula, use Relias LMS for course creation/enrollment/tracking, and partner with HR to support workforce engagement, recruitment, and retention.

Provide face-to-face crisis assessments, intervention, follow-up, intake and recovery planning, resource navigation and referrals to divert individuals from ERs, hospitals, and jails and connect them to outpatient behavioral health services.

Provide face-to-face crisis assessments, intervention, relapse prevention, and 90-day follow-up for individuals with chronic mental illness in Collin County. Deliver community-based services at residences, schools, and other safe locations 24/7. Coordinate care, develop recovery plans, advocate for clients, provide routine case management, skills training, and psychosocial rehabilitation while maintaining clear progress notes and connecting clients to community resources.
