Provide face-to-face crisis assessments, intervention, follow-up and relapse prevention for individuals with mental illness in Collin County. Deliver recovery planning, advocacy, case management, skills training, psychosocial rehabilitation and community resource education, working 24/7 across residences, schools and community locations. Coordinate services, document care, and follow clients for a 90-day period within federal, state and local guidelines.

Provide face-to-face crisis assessments, intervention, follow-up, intake and recovery/safety planning, resource navigation, referrals, and connections to outpatient behavioral health services at a diversion center, using trauma-informed, person-centered care.

Provide job coaching, skills training, job development, placement, and on-the-job support for individuals with intellectual/developmental disabilities. Conduct assessments, create individualized plans, build employer/community relationships, document progress in electronic records, and assist with life skills. Work includes travel in the community, variable hours, crisis de-escalation, and collaborative team staffing.
