Provide community-based ACT case management: assessments, recovery planning, resource navigation, referrals, supported housing/employment, crisis intervention, and linkage to medical and social services. Work collaboratively with ACT team, community partners, and fidelity managers; document services in electronic health record; respond on call as needed; promote trauma-informed, strength-based, person-centered care to reduce hospitalizations and support independent living.

Provide job coaching, skills training, job development, placement, and on-the-job support for individuals with intellectual/developmental disabilities. Conduct assessments, create individualized plans, build employer/community relationships, document progress in electronic records, and assist with life skills. Work includes travel in the community, variable hours, crisis de-escalation, and collaborative team staffing.

Provide face-to-face crisis assessments, intervention, follow-up, intake and recovery planning, resource navigation and referrals to divert individuals from ERs, hospitals, and jails and connect them to outpatient behavioral health services.
