Coordinates enrollment for Medicaid Waiver and IDD services by completing paperwork, assessing needs, developing plans, making referrals, and connecting individuals and families with medical, social, vocational, financial, and community resources. Documents services in the electronic health record, communicates with agencies and regulators, supports crisis de-escalation, addresses complaints, and provides trauma-informed, person-centered care. Builds relationships with providers and stakeholders while promoting independence, autonomy, and community participation.

Provides community-based behavioral health case management for adults on an ACT team, including assessments, recovery planning, referrals, supported housing and employment, skills and medication training, crisis intervention, psychosocial rehabilitation, and service coordination. Maintains documentation, participates in daily team huddles and on-call crisis coverage, supports access to healthcare and benefits, coordinates with community agencies, and promotes trauma-informed, person-centered recovery and independence.

Provides trauma-informed case management, skills training, medication support, and psychosocial rehabilitation to justice-involved individuals with serious mental illness or substance use disorders. Coordinates medical, social, vocational, financial, and educational services; completes assessments and service plans; documents care; manages scheduling and crisis procedures; and collaborates with criminal justice agencies to reduce recidivism and support community stability.
