Provide intensive, field-based outreach and case management to unsheltered individuals (caseload 15–20). Rapid response outreach, develop client-centered Housing First plans, coordinate MDT meetings and referrals, assist with documentation and applications, support post-housing transitions up to one year, and maintain accurate documentation and community partnerships.

Provide case management and children/youth programming for families experiencing homelessness. Create individualized plans, connect families to housing, benefits, healthcare and schools, run child-focused groups and summer camp, maintain documentation in HMIS, support site operations, travel locally, and provide trauma-informed crisis intervention and community partnership building.

Provide trauma-informed case management and outreach to unsheltered individuals, develop individualized plans, connect clients to housing, benefits, healthcare, and employment resources, maintain HMIS/client records, build community partnerships, perform crisis intervention, and support program planning and reporting.
