Develop referral growth in an assigned Houston territory by building relationships with physicians, facilities, discharge planners, home care providers, and community agencies. Create and execute quarterly and annual territory plans, meet patients and families to discuss home care needs, and coordinate with Intake and patient care staff to ensure effective communication and achievement of growth goals.

Coordinate patient visit scheduling and maintain scheduling records; verify referrals and insurance; staff cases and ensure after-hours coverage; communicate schedule changes and messages between office and field staff; escalate scheduling issues to supervisor; support Business Office Manager and participate in training.
