Provides daily living and attendant care assistance to elderly or dependent participants, including transfers, toileting, meals, transportation, housekeeping, activities, and exercise. Maintains attendance and care records using EVV software, supports center operations and cleaning, answers phones, and assists professional staff. Requires CPR and First Aid training, physical lifting ability, a driver’s license, local travel, TB testing, and background and drug screening.

Manages clinical quality improvement, patient-centered medical home compliance, risk management, EHR support, clinical outcome monitoring, chronic disease management, audits, peer review, and staff training. Coordinates QI committees, maintains NCQA PCMH requirements, supports healthcare reporting and corrective actions, and assists with patient rooming and clinical records. Requires licensed nursing or clinical credentials, healthcare quality experience, computer proficiency, and local service-area travel.

Works with eligible Veteran families to secure and retain housing, develop housing stability plans, coordinate supportive and temporary financial assistance, and maintain client records. Conducts assessments, enrollments, recertifications, housing inspections, follow-ups, referrals, and documentation reviews. Builds relationships with landlords, service agencies, employers, public assistance providers, and VA offices. Also performs community outreach and assists with marketing materials.
