The Role
Works with eligible Veteran families to secure and retain housing, develop housing stability plans, coordinate supportive and temporary financial assistance, and maintain client records. Summary: The Case Manager will be responsible for working with eligible Veteran families to obtain/retain housing,  

develop housing stability plans and assist in obtaining eligible program supportive and temporary financial services. The  

Case Manager will also routinely market and conduct program outreach activities. 


Essential Duties: 


  1. Assess participant needs to determine what program supportive and/or temporary financial services are required to stabilize the household. 

  1. Provide potential client referrals in a timely manner to the Outreach and Eligibility Specialist so they may fully screen for program eligibility. 

  1. Intake/Enroll program applicants once case has been assigned by Assistant Project Director.    

  1. Develop a housing stability plan that includes how participants will fund their housing, utilities, food, etc. after program financial assistance ends. 

  1. Assess housing units to ensure adequacy for participants and compliance with Department of Housing and Urban Development guidelines. 

  1. Work with the Housing Coordinator to establish and maintain positive relationships with local landlords/property owners in order to provide housing for clients. 

  1. Submit check requests and applicable documentation to the Assistant Project for payment processing in Blackbaud. 

  1. Maintain client data in the Homeless Management Information Systems database as well as EmpowOR. 

  1. Conduct recertification reviews every 90 days of each client household to ensure eligibility for continued services.  Also conduct periodic follow-up assessments of clients’ housing status after services have ended. 

  1. Submit charts for review to Assistant Project Director and/or Project Director at initial enrollment, after each 90-day recertification, and upon exit.  

  1. Establish and maintain positive relationships with other human service agencies and private business owners that may provide supportive and/or temporary financial services to clients. 

  1. Assist in designing marketing/outreach materials such as emails, letters, flyers, etc. outlining eligibility and services of the program. 

  1. Market and conduct program outreach activities on a routine basis to the Veteran community and establish a relationship to potential employers, public assistance programs/providers and the VA Benefits Office.’ 

  1. Perform other duties as assigned by supervisor. 

Qualifications

Education & Training: 

Bachelor’s degree preferred.  Prior experience in case management, housing-centered programs or Veteran affairs also preferred.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Position requires occasional or frequent moderate physical activity. 

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is occasionally required to stand; walk, climb, balance, stoop, kneel, crouch, crawl, and use hands to manipulate, handle or feel; and reach with hands and arms. The employee is occasionally required to lift and/or exert force up to 50 pounds. 

Other Requirements: 

Must have a valid driver’s license and the flexibility and willingness to change schedules and duties as needed according to staff availability.  Must be willing to travel the service area as needed. Travel is primarily local or within the service area during the business day, although some out-of-the-area and overnight travel may be expected.  

Agency policy regarding drug and alcohol testing and criminal background checks applies to this position. Must maintain a high level of professionalism, demonstrated by a positive attitude, ability to function as a team member, and a commitment to the overall mission of the agency. 

Skills Required

  • Bachelor's degree
