Provide street-based outreach and intensive case management to 25–30 clients with substance use disorders and low-level drug offenses. Assess needs, connect clients to housing, benefits, medical and behavioral health services, advocate within the criminal justice system, build community partnerships, and deliver education and training to expand access to medication-assisted treatment.

Assess substance use and housing needs, assist with benefits and housing access, coordinate with community and criminal justice partners, advocate in court, and represent the program at community events while providing education and training on medication-assisted treatment and harm reduction.

Provide outreach and field-based support to individuals with substance use disorder, including jail referrals, screening, care coordination, enrollment in services/grants, community relationship building, data entry, reporting, and facilitating follow-up for housing, transportation, treatment, and other recovery supports.
