Provide street and community-based outreach and peer support to individuals experiencing homelessness, substance use disorder, and justice involvement. Respond with EMS, connect clients to housing, treatment, mental health, and basic needs resources; advocate with providers and document interactions. Collaborate with partners, participate in team coordination and training, and maintain confidentiality and professional boundaries under HIPAA.

Provide outreach and intensive case management for ~25-30 participants involved in low-level drug offenses or prostitution. Conduct street and partner-site engagement, assess needs, connect clients to housing, benefits, medical and behavioral health services, collaborate with LEAD partners, advocate in criminal justice settings, and represent the program at community events and trainings.

Provide peer-based recovery support for patients with opioid use disorder: coordinate care with Hub & Spoke teams, perform screenings, provide MAT education, chart in the EMR, support self-management, conduct outreach, and facilitate contingency management and referrals to community resources.

Perform a variety of moderate, waived, and limited high-complexity diagnostic laboratory tests; run quality control, perform instrument maintenance and troubleshooting, process and ship specimens, verify and report results, maintain records, order supplies, and comply with HIPAA and other patient privacy regulations.
