The Care Navigation LPN processes hospice referrals, coordinates intake, schedules appointments through dispatch, and follows up on delayed referrals. The role performs inpatient direct admissions, handles clinical calls from patients and caregivers, resolves or escalates concerns, communicates issues to care teams, and participates in quality and process improvement initiatives.

Provides after-hours hospice nursing care to patients and families in residential settings. Responsibilities include assessing or gathering patient needs, managing pain and symptoms, providing skilled nursing, emotional support, education, referrals, physician liaison services, death visits, documentation, triage coverage, and team communication. The role requires independent judgment, home visits in varied conditions, and support for patients across all age groups.

Provides after-hours hospice nursing care in patients’ homes, including symptom monitoring, skilled nursing services, patient and family education, emotional support, physician liaison duties, death visits, referrals, and documentation. The role requires independent judgment in stressful situations, collaboration with care teams, reliable communication, and the ability to travel to residences in varying weather and physical conditions.

Provides hospice nursing care to terminally ill patients in varied settings. Assesses patient and family needs, delivers prescribed skilled nursing services, manages pain and symptoms, provides education and emotional support, coordinates with physicians and care teams, maintains clinical documentation, supervises HNAs, participates in on-call coverage, care conferences, quality improvement, bereavement follow-up, and discharge planning.

Provides after-hours hospice nursing care in patients’ residences, including assessing or gathering patient needs, managing pain and symptoms, supporting families, educating caregivers, coordinating with physicians and care teams, completing documentation, and attending death visits. RN duties additionally include patient assessment and triage coverage. Requires independent decision-making, emotional resilience, physical mobility, and the ability to respond in varied residential and weather conditions.
