Provides full-time on-call hospice nursing care, responding to urgent and routine patient needs. Conducts comprehensive physical, emotional, social, and spiritual assessments; documents care accurately in patient homes and agency systems; coordinates care; and supports families caring for terminally ill loved ones. Requires a current RN license, at least one year of medical-surgical or acute-care nursing experience, computer and EMR proficiency, and current CPR certification if required by the state.

The RN Clinical Liaison develops hospital and medical-facility referral relationships, promotes hospice services, builds census, and supports growth goals. Responsibilities include visiting physicians, case managers, discharge planners, social workers, and healthcare providers; educating patients and families about hospice; assisting with eligibility determinations; and obtaining care consents. The role requires an active registered nurse license, healthcare sales or marketing experience, hospice knowledge, relationship-building skills, and reliable transportation for the Chattanooga territory.

Provides on-call hospice nursing care for patients and families from Friday evening through Monday morning across Dayton, Rhea, and Bledsoe County. Responsibilities include responding to urgent and routine needs, conducting holistic patient assessments, documenting care accurately in home and agency settings, coordinating care, and educating and supporting families of terminally ill patients.
