Provide comprehensive primary care to a patient panel (office and telehealth), perform wellness visits and HRAs, manage chronic and acute conditions, document HCC/ICD-10 coding, collaborate with interdisciplinary teams, meet quality metrics (STARS/HEDIS), and contribute to care model improvements.

Lead analytics and strategy initiatives to drive operational and financial performance across clinics. Build complex models and reports, perform detailed analyses, govern metrics, evaluate build/buy/partner decisions, and identify generative AI opportunities while advising executive leadership and monitoring regulatory and funding changes.

Lead product strategy and execution for healthcare-focused solutions: own roadmap and success metrics, run user and stakeholder research, prioritize based on data and value, partner with engineering to deliver scalable solutions, communicate with stakeholders including executives, and coach product managers to improve product craft and outcomes.

The Welcome Coordinator will be the first point of contact for patients, managing communications, scheduling, and front office operations to ensure a warm and patient-centered experience.

Own and evolve the data platform: architect, build, and maintain data models, pipelines, and access layers. Ensure data quality, governance, and reliability; monitor and optimize infrastructure; provide technical leadership, code reviews, and collaborate with cross-functional teams to deliver scalable data solutions for operations and analytics.
