Provides nonclinical, peer-based care management to adolescents and adults with behavioral-health needs. Supports hospital-to-community transitions, coordinates referrals and resources, assesses social determinants of health, develops care plans, and connects clients with professional and community supports. Uses lived experience to promote engagement, self-advocacy, and community integration while documenting services and collaborating with an interdisciplinary team.

Provides care coordination and case management for children and young adults with mental illness or chronic medical conditions. Conducts strengths assessments, develops individualized plans, supports client goals, coordinates services, advocates for clients, facilitates provider linkages, and assists with crisis and safety planning. The role includes home visits, travel, evening hours twice weekly, documentation, incident reporting, and collaboration with mental health providers and community systems.

Supervise and support Children’s Health Homes staff, including hiring, training, onboarding, performance evaluations, scheduling, and time approval. Conduct client chart reviews, support compliance with OMH and DOH standards, coordinate with children’s SPOA, DOH, and Health Homes, and attend program meetings. The role provides care coordination oversight for children and young adults with mental illness or chronic medical conditions.
