Provide outreach, engagement, and peer support to adolescents and adults experiencing early psychosis. Provide direct case management to youth in the Home Base program, carrying a caseload of 14 with weekly home visits across Suffolk County. Deliver linkage, advocacy, referrals, motivational interviewing, parenting and anger-management support; document services per OMH/DSS/FSL requirements; collaborate with interdisciplinary teams and partner agencies; participate in System of Care initiatives.

Provide in-home family support to divert youth from the juvenile justice system: conduct assessments, case coordination, advocacy, mentoring, deliver anger management and parenting skills, maintain electronic health records, and perform home visits across Suffolk County. Requires travel and two evenings weekly.

Assist lead teacher in a Universal Pre-K classroom following OCFS and district guidelines. Support classroom activities, behavior management, assessments, record-keeping, and maintaining a safe, clean, nurturing environment while collaborating with colleagues.

Provide peer support to caregivers and families of youth with behavioral, mental health, or substance challenges. Meet weekly in homes/communities, develop treatment goals, document in EHR, transport clients as needed, collaborate with CFTSS and Care Management staff, and participate in team meetings.

Supervise clinical and administrative operations of a 24/7 outpatient crisis stabilization program including hotline and mobile response. Oversee hiring, training, scheduling, on-call rotations, compliance, staff supervision, EMR use, crisis intervention training, stakeholder coordination, and program outcome monitoring. Provide clinical backup and ensure staff and client safety.

Manage daily clinic operations at a behavioral health recovery center: supervise staff, oversee front office with high volume, maintain EMR data integrity, handle supplies, facility maintenance, vendor coordination, technology operations, scheduling, and client complaint resolution.

Provide intensive case management for 40–50 clients with behavioral health, medical, or substance use diagnoses. Conduct outreach, meet DOH visit requirements, maintain EHR clinical documentation, coordinate care with providers, complete timely reporting, and perform home and community visits as needed.

Provide front-desk administrative support at the Family Recovery Center: answer phones, greet and sign in clients, schedule appointments, enter data into OASAS, verify insurance, take payments, run ISTOPS, make reminder calls, perform scanning and deposits, and assist clients navigating clinic services.

The Recreational Aide assists with educational, recreational, and enrichment programs for children in emergency housing, providing supervision and support.

The Administrative Assistant will provide administrative support, interact with clients, manage scheduling, maintain records, and assist with paperwork at the Family Recovery Center.

The Case Manager provides care management for behavioral health needs, collaborating with a multidisciplinary team to create and manage care plans while facilitating community integration and support.

The Case Manager will support families in early childhood development through home visits, focusing on literacy and parent-child interaction.
