The role requires occasional local travel and attendance at signature evening and daytime events.

Provides in-home family support and direct services to youth and parents in the Alternatives for Youth Program. Responsibilities include assessments, service coordination, advocacy, mentoring, conflict management, anger management, parenting-skills training, community-resource linkages, electronic health-record documentation, and home visits throughout Suffolk County to help divert youth from the juvenile justice system.

Provides comprehensive case management for senior citizens, including intake, assessment, service planning, care coordination, system navigation, outreach, and follow-up. Connects clients with health, social, financial, government, and community services; coordinates transportation and obtains consent forms. Develops recreational, educational, and social activities while conducting home visits and supporting access to needed resources.
