Provide psychiatric evaluations, diagnosis, and treatment for adults in skilled nursing facilities. Collaborate with interdisciplinary teams, consult with APRNs, submit documentation within 24 hours, adhere to state SNF regulations, and perform PEC and PC370 evaluations as needed.

Provide clinical, clerical, and administrative support for home care interdisciplinary team. Duties include scheduling and assigning cases, monitoring RN supervisory visit compliance, triaging calls, coordinating care, documenting interactions, auditing paperwork, reviewing DME/pharmacy use, handling complaints, and assisting supervisors with administrative tasks.

Provide bereavement assessment, one-to-one and group counseling, follow-up for families up to one year post-death, maintain records, provide referrals, run support groups and memorial events, and expand community resources.

Provide skilled hospice nursing care in patients' homes, assisted living, and SNFs. Perform comprehensive assessments, coordinate interdisciplinary plans, document care in an electronic system, and communicate status changes to supervisors and physicians. Participate in team meetings and provide weekend/holiday coverage as needed.

Perform clinical documentation audits, manage and analyze QAPI quality data, maintain dashboards and regulatory submissions (SHP, CMS HOPE), support PIPs and accreditation readiness, serve as backup pre-bill auditor, and collaborate with clinical, compliance, coding, education, and operations teams to ensure hospice regulatory compliance and quality improvement.

Provide school-based behavior analytic assessment, programming, implementation, and supervision for students with ASD; conduct formal assessments, train and supervise technicians, maintain documentation, and collaborate with school teams.

Provide clinical leadership and supervision for a Certified Home Health Program, overseeing multidisciplinary home-based care, ensuring OASIS, state and Medicare compliance, staffing, documentation accuracy, recruitment, and on-call/coverage coordination across the North Shore Suffolk area.
