Provide skilled home-based nursing to adult and geriatric patients: conduct head-to-toe assessments, manage medications and wound care, complete OASIS assessments, develop and coordinate Plans of Care, perform admissions/recertifications/discharges, educate patients and caregivers, document using iPad, and report to Clinical Manager.

Provide school-based occupational therapy services to children: conduct evaluations, deliver individual and small-group interventions, collaborate with school teams and families, document progress, and participate in IEP meetings. Average caseload ~12 students, aligned with the school calendar.

Oversee and coordinate all clinical hospice services, supervise clinical staff and supervisors, ensure regulatory compliance and clinical record-keeping, support budget and operational planning, drive quality improvement and performance measurement, and develop organizational policies to ensure safe, compassionate patient- and family-centered care.

Supervise and coordinate multidisciplinary home health services (nursing, PT, OT, SLP, social work, home health aides). Lead and evaluate staff, ensure accurate documentation, maintain regulatory and Medicare compliance, manage staffing and on-call coverage, participate in hiring and training, and support high-quality patient care in the home setting.

Provide supervised, in-home physical therapy services per physician Plan of Care. Assess, implement, and document PT treatments, teach patients/families, coordinate with care team, and report progress to physicians. Maintain clinical records and confer with supervising PT at least every 30 days.

Provide school-based occupational therapy assistance to children including evaluations, individual and small-group interventions, documentation, and participation in IEP meetings while collaborating with school teams and families.

Develop, implement, and manage behavior analytic programs for home-based clients; conduct assessments (e.g., VB-MAPP, Vineland-3), make clinical recommendations for ASD, maintain documentation, supervise and train behavior technicians, consult with schools, provide direct services when needed, and complete timely reports and trainings.

Provide BCBA services including assessment (VB-MAPP, Vineland, etc.), develop and monitor behavior-analytic programs, supervise and train behavior technicians, maintain documentation, consult with schools, and ensure program materials and compliance.

Provide direct support to students with autism in self-contained elementary classrooms. Assist with IEP and behavior plan implementation, daily living skills, behavior management, transitions, data collection, and collaboration with teachers and related service providers to promote student independence.

Provide development, implementation, evaluation, and management of home-based behavior analytic programs for clients with ASD. Supervise and train behavior technicians, conduct assessments (VB-MAPP, Vineland-3, etc.), maintain documentation, consult with schools, and collaborate with BCBA colleagues to ensure clinical quality and compliant reporting.

Provide psychiatric evaluations, diagnosis, and treatment for adults in skilled nursing facilities. Collaborate with interdisciplinary teams, consult with APRNs, submit documentation within 24 hours, adhere to state SNF regulations, and perform PEC and PC370 evaluations as needed.
