Field-based Registered Nurse Case Manager providing skilled nursing, assessments, wound care, medication oversight, OASIS assessments, admissions/recertifications/discharges, care plan development, patient and caregiver education, and timely documentation using an iPad while covering an assigned territory.

Lead and supervise clinical hospice personnel, coordinate patient and family care, ensure regulatory compliance and quality of services, assist with budgeting and performance improvement, and support clinical supervisors in team management and planning.

Provide BCBA services including assessment (VB-MAPP, Vineland, etc.), develop and monitor behavior-analytic programs, supervise and train behavior technicians, maintain documentation, consult with schools, and ensure program materials and compliance.

Provide school-based occupational therapy services to children, including evaluations, individual and small-group interventions, IEP meeting participation, documentation, and collaboration with school teams and families.

Provide clinical leadership and supervision for a multidisciplinary home health team, ensuring quality patient care, compliance with state and Medicare standards, staffing coordination, staff recruitment and performance management, on-call coverage, and accurate documentation including OASIS assessments.

Provide school-based behavior analytic assessment, develop and implement individualized programs, monitor and modify treatment, supervise behavior technicians, complete required documentation, collaborate with school teams, and conduct staff training.

Provide skilled physical therapy services in patients' homes under a supervising Registered Physical Therapist. Implement and update Plan of Care, teach and supervise patients and families, coordinate with care team and physicians, document clinical notes, and report patient progress.

Provide development, implementation, evaluation, and management of home-based behavior analytic programs for clients with ASD. Supervise and train behavior technicians, conduct assessments (VB-MAPP, Vineland-3, etc.), maintain documentation, consult with schools, and collaborate with BCBA colleagues to ensure clinical quality and compliant reporting.

Provide direct support to students with autism in self-contained elementary classrooms. Assist with IEP and behavior plan implementation, daily living skills, behavior management, transitions, data collection, and collaboration with teachers and related service providers to promote student independence.

Develop, implement, and manage behavior analytic programs for home-based clients; conduct assessments (e.g., VB-MAPP, Vineland-3), make clinical recommendations for ASD, maintain documentation, supervise and train behavior technicians, consult with schools, provide direct services when needed, and complete timely reports and trainings.

Provide psychiatric evaluations, diagnosis, and treatment for adults in skilled nursing facilities. Collaborate with interdisciplinary teams, consult with APRNs, submit documentation within 24 hours, adhere to state SNF regulations, and perform PEC and PC370 evaluations as needed.

Provide school-based speech-language services including screening, comprehensive evaluations, IEP participation, intervention planning and delivery, documentation, consultation, supervision/mentoring, transition planning, and travel between multiple school or home locations. Ensure compliance with school team processes and productivity expectations.
