The LVN I is responsible for managing medication protocols, conducting patient support, and administration tasks while collaborating with various healthcare programs and ensuring compliance with medical procedures.

Provide nursing support in an adult mental health outpatient clinic: manage medication protocols and inventory, administer Prescription Assistance Program, support telemedicine and primary care programs, and assist other programs as assigned. Follow physician orders, maintain records, and ensure medication cost savings while working collaboratively with clinical teams.

Provide community-based case management and wraparound facilitation for at-risk individuals and families: assessment, treatment planning, service coordination, crisis prevention/management, skills training, referrals, and monitoring within a recovery-based framework. Serve as point of contact when supervisors are unavailable.

Provide case management services including assessment, treatment planning, monitoring, crisis prevention/response, skills training, referrals, and resource coordination for children, adolescents, and families within a recovery-focused framework. Maintain records, collaborate with schools and systems, and assist clients in accessing medical, social, and educational services while working toward required trainings and certification.
