Provide recovery-focused case management and community rehabilitation to individuals with severe, persistent mental illness in site-based permanent supportive housing. Coordinate assessments and interdisciplinary service plans, assist with daily living skills, crisis intervention, community integration, appointments, documentation, and resident rule compliance. Participate in meetings, training, and on-call emergency support.

Provide time-limited and ongoing recovery-focused case management for newly opened clients, coordinating assessments, treatment planning, benefits and housing assistance, crisis intervention, community rehabilitation, and transition to regional teams while maintaining documentation and agency standards.

The Insurance Verification Specialist verifies insurance eligibility, registers new clients, collects payments, and collaborates with healthcare staff for accurate client information.

Provide psychiatric evaluations, medication management, and somatic therapies for severely mentally ill adults. Lead interdisciplinary treatment teams, consult with staff, coordinate physical health care and referrals, monitor labs and medication side effects, and perform involuntary psychiatric evaluation certifications. Participate in clinic administration, documentation, and quality review to support program effectiveness.

Provide recovery-focused community rehabilitation and case management for individuals with substance use and/or severe mental illness. Coordinate assessments, develop and monitor Interdisciplinary Service Plans, connect clients to community resources, provide outreach, advocacy, crisis assistance, accompany clients to appointments, ensure housing occupancy compliance, maintain documentation, and collaborate with a multidisciplinary team.

Coordinate, design, deliver, and evaluate competency-based training and onboarding across the organization. Maintain LMS and CE Broker, mentor new case management staff, support CARF and regulatory training requirements, manage training committees, and produce training reports to support quality, compliance, and staff development.

The Receptionist will greet clients and visitors, manage registration and scheduling, operate a switchboard, and ensure supply availability.

The Payee Associate manages client finances, schedules disbursements, maintains accounts, prepares reports, and coordinates with Social Security for client activities under supervision.

The Accountant III prepares financial statements, maintains the general ledger, assists with financial reporting and analysis, and coordinates various financial activities within the Finance Team.
