Teams at ClareMedica

Provides comprehensive primary care to adult and senior Medicare Advantage patients, managing chronic conditions, preventive care, care gaps, HEDIS and Star measures. Coordinates with clinical teams, documents accurately using EMR systems, supports quality improvement, performs HCC coding, and maintains compliance, licensure, and certifications. Occasional travel to nearby care centers may be required.

Supports senior patients in accessing Medicaid, Medicare Savings Programs, SNAP, LIHEAP, housing, transportation, and other community benefits. Screens eligibility, assists with applications and recertifications, conducts member orientations, generates leads, tracks engagement, documents services in the EHR, and collaborates with clinical, care management, sales, and community teams. Local travel between care centers may be required.
