Provide comprehensive primary care to adult and senior Medicare Advantage patients, including preventive care, chronic disease management, wellness visits, care coordination, and health education. Drive HEDIS, Star Ratings, and care-gap performance while maintaining accurate EMR documentation and HCC coding. Collaborate with clinical teams, case managers, and risk adjustment specialists, participate in quality improvement and compliance activities, and maintain required licensure, certifications, and CME.

Provides independent nurse practitioner care for clinic patients, including histories, physical examinations, diagnosis, treatment planning, prescribing, ordering and interpreting tests, monitoring interventions, referrals, and patient education. Works with multidisciplinary teams under an established physician protocol and applicable regulations. The role requires frequent local travel, driving, occasional overnight travel, and bilingual Spanish communication.

Provides independent nurse practitioner care, including patient assessments, diagnoses, treatment planning, prescribing, diagnostic testing, interventions, monitoring, referrals, and patient education. Requires collaboration with multidisciplinary healthcare teams and practice under established medical protocols. The role involves local and occasional overnight travel, frequent driving, and conducting sales presentations.
