Supports senior patients in accessing Medicaid, Medicare Savings Programs, SNAP, LIHEAP, housing, transportation, and other community benefits. Screens eligibility, assists with applications and recertifications, conducts member orientations, generates referrals, advocates for patients, documents interactions in the EHR, and collaborates with clinical teams, care managers, sales staff, and community agencies. Local travel between care centers may be required.

The Sales Manager oversees and coaches sales agents, establishes performance goals, monitors sales and center metrics, ensures compliance with Medicare and company policies, and provides onboarding and recurring training. The role also develops relationships with health plans, brokers, providers, community organizations, and other partners. Local travel between care centers may be required.
