Provide medical care to residents in skilled nursing facilities and senior living communities in North Texas. Manage patient treatment plans, coordinate with clinical staff, handle inpatient/post-acute care, and optionally serve in Medical Directorship roles. Maintain documentation, collaborate with care teams, and uphold quality standards.

Provide medical care to residents in skilled nursing facilities and senior living communities across North Texas. Provide in-person medical care to patients in skilled nursing and rehab facilities, manage assessments and treatment plans, reduce readmissions, collaborate with facility teams, participate in value-based/transitional care programs, and document in the EMR.

Manage daily operations of a post-acute care practice across SNFs, rehab, assisted living, and long-term care. Oversee KPIs, staffing, scheduling, documentation, onboarding for providers and facilities, develop workflows and policies, mentor staff, and support facility relationships and issue resolution to optimize care delivery.
