Provides in-person medical care to post-acute patients in skilled nursing and rehabilitation facilities. Responsibilities include patient assessments, treatment planning, EMR documentation, chronic and acute condition management, readmission reduction, transitional care, and collaboration with facility and clinical teams. The role requires independent clinical judgment, ongoing facility presence, and coordination with staff to support quality outcomes.

Manage daily operations of a post-acute care practice across SNFs, rehab, assisted living, and long-term care. Oversee KPIs, staffing, scheduling, documentation, onboarding for providers and facilities, develop workflows and policies, mentor staff, and support facility relationships and issue resolution to optimize care delivery.

Provide in-person medical care to patients in skilled nursing and rehabilitation facilities, manage assessments and treatment plans, document in EMR, collaborate with facility teams to reduce readmissions, support value-based/transitional care programs, and maintain an on-site presence per schedule.

Provides evidence-based education and professional development for nursing, interdisciplinary, and non-licensed clinical staff in post-acute care facilities. Designs learning activities, addresses practice gaps, evaluates outcomes, supports competency, serves as a clinical resource, collaborates with medical and organizational stakeholders, and promotes practice change. Travels throughout the Greater Houston area to support clinical staff.

Provides direct medical care to residents in assisted and independent living communities throughout the Dallas-Fort Worth area. The physician works part-time or full-time with flexible scheduling, minimal call, administrative support, malpractice coverage, and benefits. Candidates must hold an active Texas medical license and DEA certification and be board certified. Inpatient or hospitalist experience is welcomed but not required.
