Provides clinical and residential support for clients with substance use or mental health needs. Responsibilities include treatment-plan support, crisis intervention, conflict management, family visits, group facilitation, case management assistance, resident monitoring, toxicology screening, medication observation, psychoeducation, emergency procedures, and detailed electronic documentation.

Provides reception and clerical support for the HEAP program. Responsibilities include answering inquiries, screening applications, processing mail, maintaining records and filing systems, tracking supplies, greeting visitors, making referrals, logging site visitors, and supporting program staff while maintaining confidentiality and a trauma-informed environment.

Provides non-medical in-home support to program participants, including housekeeping, personal care, meal preparation, supervision, safety monitoring, community appointments, activities, documentation, and administrative program support. Maintains accurate records, communicates client concerns, assists with meetings and events, and supports a clean, positive shared environment. Requires a valid New York State driver’s license, vehicle access, and the ability to lift 20 pounds.

Leads annual giving, direct mail, corporate partnership, donor cultivation, stewardship, and fundraising event strategies. Supervises donor relations and development staff, manages campaign budgets, identifies high-value prospects, prepares donor communications and proposals, tracks financial progress, and supports development planning. The role also makes donor solicitations, builds recurring and major-gift portfolios, coordinates corporate benefits, and collaborates with development and marketing teams to increase contributions.

Lead development, launch, and management of the Steps to Success Mobility Mentoring program. Oversee staffing, grant administration, budgeting, compliance, community engagement, stakeholder relations, and performance reporting. Provide strategic vision, supervise program managers, drive client recruitment, and ensure culturally competent, data-informed service delivery to achieve program outcomes.

Supervise and oversee Self-Directed Services, serving as liaison among participants, families, care managers, brokers, and regional offices. Manage fiscal intermediary functions, budgets, payroll, reimbursements, invoices, Medicaid billing, documentation, compliance, staff training, and participant support plans. Provide brokerage services, monitor health and safety, supervise self-directed staff when needed, support incident and fraud investigations, and ensure regulatory compliance for assigned participants.

Provides therapeutic counseling, case management, assessments, advocacy, referrals, and parent education to kinship families. Conducts primarily in-home visits, facilitates support groups, develops community activities, coordinates services with county agencies, documents interventions, completes evaluations, and supports trauma-informed care. Requires collaboration with supervisors and community partners, ongoing training, professional development, and compliance with agency, privacy, and documentation standards.

Provide individual and group peer support for clients with substance use disorder, lead recovery education sessions, assist clients with recovery plans, transport clients, complete required documentation, participate in trainings and agency meetings, and support a trauma-informed, collaborative care environment.

Manage and supervise nursing services across OPWDD residential programs, provide direct medical care and oversight, ensure documentation and medication compliance, coordinate training and orientation, collaborate with clinical teams and community providers, use QA data to maintain high-quality, trauma-informed care.
