Provides compassionate support and scheduled services to senior residents, documents care, completes incident reports, responds to help and pull cords, and assists the nursing team. The MDS Coordinator oversees and completes MDS assessments, RAI documentation, and resident care planning in a 72-bed long-term care and TCU facility. Responsibilities include assigning nursing tasks, developing care plans, supporting Medicare scheduling and billing, issuing non-coverage notices, maintaining records, and collaborating with admissions, case management, and supply staff. The role requires a Minnesota RN license, Resident Assessment Coordinator certification, MDS experience, and computer proficiency.

Greet and welcome residents and visitors, answer and route multi-line phone calls, handle basic administrative tasks and recordkeeping, assist with word-processing and basic tech questions, sort mail, and provide exceptional customer service in a senior living community. Part-time role with every-other-weekend and on-call availability.

Manage billing and collections for skilled nursing and assisted living: generate and submit claims, post cash and ancillaries, produce private pay statements, resolve billing issues, meet days-outstanding targets, perform month-end closing, and use DDE, MN-ITS, Excel and payer websites to support accurate, timely reimbursement.

Provide high-quality PT/OT/ST services to older adults and other patients: assess, develop and implement individualized treatment plans, collaborate with interdisciplinary teams, document care, and ensure regulatory compliance.
