Care Initiatives

Care Initiatives is an Iowa-based nonprofit senior-care provider focused on improving quality of life during health transitions through compassionate, individualized care. It operates skilled nursing, long-term care, rehabilitation, assisted living, independent living, and hospice services. Its earnings are reinvested in team-member education, its care locations, new facilities, and programs. Founded in 1989, the organization serves residents, patients, and families across Iowa and expanded hospice care into Omaha, Nebraska, in 2024.
