Provides skilled nursing care to patients in their homes under physician orders and Clinical Manager supervision. Conducts assessments, develops and updates care plans, delivers treatments, documents visits, coordinates interdisciplinary services, supervises LPN/LVN and home health aide personnel, educates patients and families, completes OASIS assessments and clinical documentation, responds to changes in patient condition, participates in quality reviews, and performs on-call duties as assigned.

Provides physical therapy services to patients in their homes on an intermittent basis, following physician orders and the established plan of care. Works under the direction and supervision of a Clinical Manager, Therapy Supervisor, and supervising Physical Therapist. Performs delegated therapy services while exercising initiative and independent judgment and adhering to agency philosophy and goals.

Provides intermittent skilled nursing care to patients in their homes under physician orders and RN supervision. Administers medications and treatments, performs preventive and rehabilitative procedures, monitors patient conditions, documents care, teaches patients and families, supports care-plan updates, and reports changes or incidents. Maintains confidentiality, participates in audits and care conferences, follows aseptic technique and agency policies, and maintains current CPR certification.

Provides intermittent occupational therapy to home-based and hospice patients under physician orders and the supervision of an Occupational Therapist and Clinical Manager. The role involves delivering patient care, communicating with patients, families, physicians, nurses, and healthcare agencies, following care plans and healthcare regulations, documenting activities, and working independently or collaboratively. Candidates must hold current state licensure and CPR certification and pass a background check.

Evaluates patient function, develops and updates care plans, delivers skilled therapy, documents clinical progress, consults with physicians and families, coordinates with healthcare personnel, and participates in care conferences, staff meetings, and in-service programs.
