Provide physical therapy interventions in patients' homes under the supervision of a licensed Physical Therapist, following physician orders and plan of care. Document treatments, exercise initiative and independent judgment, align with agency philosophy, and support clinical manager/therapy supervisor direction.

Provide skilled, physician-ordered speech-language pathology services in patients' residences. Complete evaluations, develop and submit plans of care, document progress, consult with physicians and family, participate in care planning and in-service training, and adhere to agency policies.

Support the Director of Marketing by developing referral relationships, driving market business development for Home Care/Hospice, coordinating with clinical staff, creating marketing plans, managing pharmacy account transitions, and producing weekly, monthly, and annual marketing reports. Role requires schedule flexibility, potential after-hours work, and significant travel (up to 90%).
