Manage branch office communications and clerical functions: answer phones, maintain CNA/PCA schedules and care plans, monitor patient charts and records, track physician orders and logs, and perform miscellaneous office tasks to support coordination of home health and hospice care.

Provides skilled intermittent nursing care to patients in their homes under physician orders and Clinical Manager supervision. Conducts assessments, develops and revises care plans, performs specialized nursing procedures, documents visits and OASIS assessments, coordinates interdisciplinary services, educates patients and families, supervises nursing aides and LPNs, manages on-call duties, and maintains CPR certification and regulatory compliance.

Provides hospice on-call nursing support during evenings, nights, weekends, and some holidays. Responds promptly to calls, triages patients, applies critical-thinking skills, navigates electronic medical records, and supports hospice patients within Salt Lake County. Requires a current Utah RN license, CPR certification, reliable transportation, auto insurance, and home health or hospice experience.

Provide physical therapy interventions in patients' homes under the supervision of a licensed Physical Therapist, following physician orders and plan of care. Document treatments, exercise initiative and independent judgment, align with agency philosophy, and support clinical manager/therapy supervisor direction.
