Provide physical therapy interventions in patients' homes under the supervision of a licensed Physical Therapist, following physician orders and plan of care. Document treatments, exercise initiative and independent judgment, align with agency philosophy, and support clinical manager/therapy supervisor direction.

Provide physical therapy services in patients' homes per physician orders and plan of care, under supervision of the Clinical Manager/Therapy Supervisor and supervising Physical Therapist.

Provides skilled nursing care in patients' homes according to physician orders and the plan of care. Performs initial and ongoing assessments, develops and revises care plans, documents visits and OASIS assessments, coordinates with IDT and physicians, supervises LPNs/HHAs, provides patient/family teaching, maintains CPR certification, and participates in quality, utilization review, and on-call duties.

Provide skilled, physician-ordered speech-language pathology services in patients' residences. Complete evaluations, develop and submit plans of care, document progress, consult with physicians and family, participate in care planning and in-service training, and adhere to agency policies.

Provides intermittent skilled nursing care in patients' residences per physician orders and agency care plans. Performs evaluations, develops and revises care plans, documents visits, coordinates with IDT and physicians, supervises LPNs/LVNs and HHAs, completes OASIS assessments, maintains CPR certification, and accepts on-call duties including nights, weekends, and holidays.

Full-time on-call hospice Registered Nurse providing after-hours patient triage and care during evenings, nights, weekends, and some holidays. Travel within Salt Lake County with mileage reimbursement. Use electronic medical records, respond promptly to calls, apply critical thinking, and work as part of a supportive hospice team. Requires current Utah RN license and CPR.

Provides skilled intermittent nursing care in patients' homes per physician orders and plan of care. Conducts evaluations, develops and revises care plans, documents visits, coordinates with interdisciplinary team, supervises LPNs and aides, completes OASIS assessments, participates in quality management, and takes on-call shifts nights/weekends/holidays.

Oversee home care and/or hospice clinical services, supervise and develop patient care staff, coordinate interdisciplinary care and referrals, ensure regulatory compliance, implement and evaluate individualized plans of care, support QAPI and infection control, assist with staffing, documentation, investigations, and community education.
