Provides skilled nursing care to patients in their homes under physician orders and a clinical manager’s supervision. Conducts assessments, develops and revises care plans, performs specialized nursing procedures, documents patient progress, coordinates interdisciplinary services, supervises nursing and aide personnel, educates patients and families, manages certifications and discharges, and reports changes in patient condition. Maintains CPR certification, confidentiality, regulatory compliance, clinical competency, and timely documentation.

Provides skilled nursing care to patients in their homes under physician orders and Clinical Manager supervision. Conducts assessments, develops and updates care plans, delivers treatments, documents visits, coordinates interdisciplinary services, supervises LPN/LVN and home health aide personnel, educates patients and families, completes OASIS assessments and clinical documentation, responds to changes in patient condition, participates in quality reviews, and performs on-call duties as assigned.

Provides physical therapy services to patients in their homes on an intermittent basis, following physician orders and the established plan of care. Works under the direction and supervision of a Clinical Manager, Therapy Supervisor, and supervising Physical Therapist. Performs delegated therapy services while exercising initiative and independent judgment and adhering to agency philosophy and goals.
