Provide physical therapy interventions in patients' homes under the supervision of a licensed Physical Therapist, following physician orders and plan of care. Document treatments, exercise initiative and independent judgment, align with agency philosophy, and support clinical manager/therapy supervisor direction.

Provide physical therapy services in patients' homes per physician orders and plan of care, under supervision of the Clinical Manager/Therapy Supervisor and supervising Physical Therapist.

Provides skilled nursing care in patients' homes according to physician orders and the plan of care. Performs initial and ongoing assessments, develops and revises care plans, documents visits and OASIS assessments, coordinates with IDT and physicians, supervises LPNs/HHAs, provides patient/family teaching, maintains CPR certification, and participates in quality, utilization review, and on-call duties.
