Provides full-time on-call nursing services for home health and hospice patients during evenings, nights, weekends, and some holidays. Responsibilities include responding promptly to calls, triaging patient needs, applying critical thinking, navigating electronic medical records, and delivering compassionate customer-focused care. The role requires Idaho and Oregon RN licenses, CPR certification, home health or hospice experience, reliable transportation, and auto insurance.

Provides intermittent occupational therapy to home-based and hospice patients under physician orders and the supervision of an Occupational Therapist and Clinical Manager. The role involves delivering patient care, communicating with patients, families, physicians, nurses, and healthcare agencies, following care plans and healthcare regulations, documenting activities, and working independently or collaboratively. Candidates must hold current state licensure and CPR certification and pass a background check.

Provides intermittent occupational therapy to patients in their homes under physician orders and clinical supervision. Evaluates functional needs, develops and updates care plans, performs skilled therapy, documents clinical progress, completes OASIS assessments and discharge summaries, communicates with physicians and families, supervises assistants, teaches home programs and activities of daily living, and participates in quality management activities.
