Patrol hospital buildings and grounds to prevent fires, theft, vandalism, and unauthorized entry. Conduct repetitive security tours, assess situations, follow written and oral instructions, and support a safe, caring hospital environment. The full-time position runs from 12:00 PM to 8:15 PM and requires a valid New York State Security License, driver’s license, and BLS certification.

Supports regulatory compliance and quality assurance activities for home care and hospice services. Responsibilities include collecting, aggregating, analyzing, and entering monitoring data; assisting with regulatory and departmental monitoring; and supporting education and compliance initiatives. The role requires accurate data entry, knowledge of OASIS and ICD coding, strong organization, attention to detail, and effective written and verbal communication.

Support Home Care and Hospice QA&I by collecting, aggregating, and entering survey and QA/PI data. Develop data collection tools, maintain confidential records, prepare CAHPS mailings, calculate response rates, coordinate telephone surveys, take meeting minutes, maintain employee education records, and notify Medicare D plans on admissions/discharges.

Under pharmacist supervision, receive, compound, and dispense medications per practitioner orders. Communicate with department staff, demonstrate math aptitude, read handwriting, identify drugs, and handle unit-dose products with dexterity.

Assess referred patients for hospice admission, educate patients and caregivers, generate and respond to referrals, coordinate with physicians and the interdisciplinary team, develop initial care plans, perform outreach to referral sources, and travel to patient locations including weekends.

The Vice President of Quality and Safety oversees quality management and patient safety programs, ensuring compliance with regulatory standards in hospice care.

Responsible for continuous patient care as directed by the Medical Director. Requires extensive experience in hospice and palliative care settings.

The Program Director will oversee the nursing care delivery integration, shared governance model, data analysis, and ensure evidence-based nursing practice.
