Provides on-call hospice nursing care under RN direction across homes, nursing facilities, and clinics. Responsibilities include medication and symptom management, patient and family education, documentation in MatrixCare, equipment and supply coordination, crisis response, dressing changes, progress reporting, and weekend or holiday visits. Requires travel throughout the service area, including adjacent counties, and occasional HHA support.

Leads community educators and clinical hospital liaisons while driving referral growth, sales performance, account development, and healthcare relationship management. Supervises hiring, coaching, performance reviews, scheduling, and team development. Builds relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, and other referral partners. Analyzes CRM, referral, activity, and market data, implements performance improvement plans, supports ethical marketing compliance, and collaborates cross-functionally to improve patient access.

Provides charge nursing leadership for an eight-bed hospice inpatient unit, including staff assignments, patient rounds, care coordination, quality oversight, symptom and pain management, interdisciplinary collaboration, and support for patients and families experiencing end-of-life care. The nurse also guides staff, manages supplies and equipment, addresses performance concerns, communicates with providers, and responds to patient and family crises.

Provides hospital-based hospice liaison support by responding to referrals, educating staff, coordinating patient services and admissions, supporting discharge planning, and assisting patients and families through end-of-life care. Builds relationships with hospital case managers and providers, conducts referral visits, hosts educational sessions, documents outreach in a CRM, and collaborates with interdisciplinary hospice teams.

Serve as the first point of contact for hospice, home health, and palliative care referrals. Triage incoming referral calls, verify patient and insurance information, coordinate admissions and appointments, maintain EMR records, order equipment, communicate with clinical and community partners, and convert referrals into active admissions while ensuring timely, compassionate service.

Provides after-hours hospice and palliative nursing care for patients and families across an eight-county service area. Responsibilities include direct patient care, symptom management, pain medication titration, crisis response, care-plan updates, clinical documentation, caregiver education, interdisciplinary coordination, and supervision of LPNs and hospice aides as needed. The role supports various hospice situations, including admissions, discharges, transfers, death events, continuous home care, triage, and inpatient care, on a seven-days-on, seven-days-off schedule.

Provide nursing support and triage for patients, families, and interdisciplinary teams; document EMR communications, review charts, coordinate admissions and follow-ups, and ensure hospice plan of care compliance.

Provide hospice-focused homecare nursing including patient assessments, individualized care plans, symptom management, family education and support, supervise aides/LPNs, coordinate with interdisciplinary team, and maintain timely electronic medical records.

Provide expert hospice nursing on an inpatient unit: assess and manage pain and complex symptoms, administer medications and comfort measures, document care, collaborate with interdisciplinary team, educate and support patients and families, and respond to clinical changes to promote dignity and quality of life.

Provide skilled, patient-centered hospice nursing care in patients' homes under RN direction. Monitor symptoms, assist with medication management, document visits in the EMR, communicate changes to the interdisciplinary team, educate patients and caregivers, assist with care planning, and support RN Case Managers to ensure patient comfort and safety.
