Teams at Big Bend Hospice

Provides on-call hospice nursing care under RN direction across homes, nursing facilities, and clinics. Responsibilities include medication and symptom management, patient and family education, documentation in MatrixCare, equipment and supply coordination, crisis response, dressing changes, progress reporting, and weekend or holiday visits. Requires travel throughout the service area, including adjacent counties, and occasional HHA support.

Leads community educators and clinical hospital liaisons while driving referral growth, sales performance, account development, and healthcare relationship management. Supervises hiring, coaching, performance reviews, scheduling, and team development. Builds relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, and other referral partners. Analyzes CRM, referral, activity, and market data, implements performance improvement plans, supports ethical marketing compliance, and collaborates cross-functionally to improve patient access.

Provides charge nursing leadership for an eight-bed hospice inpatient unit, including staff assignments, patient rounds, care coordination, quality oversight, symptom and pain management, interdisciplinary collaboration, and support for patients and families experiencing end-of-life care. The nurse also guides staff, manages supplies and equipment, addresses performance concerns, communicates with providers, and responds to patient and family crises.
