Provides clinical assessment, risk and safety planning, treatment planning, and recovery-based services for adults with serious mental illness. Leads care coordination for assigned clients, oversees individual treatment teams, and develops, implements, evaluates, and revises treatment goals in collaboration with multidisciplinary staff.

Provides nursing care to adults in a 24-hour community crisis stabilization program. Responsibilities include monitoring medication self-administration, assessing physical health needs, coordinating acute medical care, facilitating wellness groups, delivering recovery-oriented interventions, supporting Wellness Recovery Action Plans and safety plans, completing clinical documentation, and collaborating with a multidisciplinary team.

Provide mobile crisis services for adults, children, and families through telephone triage, crisis evaluations, risk assessments, counseling, stabilization interventions, referrals, and disposition planning. Conduct assessments in onsite and community settings, develop crisis intervention plans, document services, consult with supervisors and external providers, and provide short-term follow-up therapy and support.

Provides peer-based mental health support in an evening living-room model program. Responsibilities include connecting with clients, encouraging open dialogue, modeling recovery, advocating for peers, building supportive relationships, facilitating recovery-oriented activities, and guiding clients to additional mental health resources. The part-time schedule is 20 hours per week. Candidates must have lived experience with severe and persistent mental illness and obtain Peer Specialist Certification within six months of hire.

Provides overnight nursing care to adults in a 24-hour behavioral health crisis stabilization program. Responsibilities include monitoring medication self-administration, assessing physical health needs, coordinating acute care, facilitating wellness activities, delivering recovery-oriented nursing interventions, supporting WRAP and safety plans, completing clinical documentation, and participating in training.

The Recovery Specialist provides clinical assessment, develops treatment plans, coordinates a treatment team, and ensures comprehensive recovery-based services for adults with serious mental illness.

Provide individual and group evidence-based intensive outpatient counseling for individuals with substance use and co-occurring mental health disorders; coordinate care with multidisciplinary team, community providers, PCPs and support resources; document services in EHR and assist clients in accessing community supports.

Provide outpatient individual and group therapy within a multidisciplinary team. Manage a diverse caseload, deliver evidence-based treatments (e.g., DBT, CBT), document care in electronic health records, attend monthly trainings, and coordinate care for adults, children, and families.
