Provide primary and consultative medical care to geriatric patients at an FQHC, lead an interdisciplinary SAGE team, screen for frailty, cognitive decline and polypharmacy, manage acute and chronic conditions within NP/PA scope, document in EHR, coordinate with specialists and community resources, mentor staff, participate in quality improvement, and assist with clinical operations and protocol development.

Provide primary and consultative geriatric care, lead an interdisciplinary SAGE team, perform screenings for frailty and cognitive decline, manage polypharmacy and complex conditions, coordinate with specialists and community resources, maintain EHR documentation, ensure FQHC compliance, mentor staff, and develop geriatric care protocols.

Manage IT infrastructure, operations, helpdesk, cybersecurity, compliance (HIPAA), vendor and project management, budgeting, staff supervision, training, and support of EHR/dental practice systems to ensure reliable, secure technology services.

Provide clinical supervision, training, and oversight for Specialty Mental Health staff and interns; oversee the Behavioral Health Training Academy; monitor program operations, productivity, and contract deliverables; perform chart reviews; maintain a small clinical caseload and at least a 5 FTE supervisee load; liaise with funders and community partners; participate in quality improvement and agency activities.

Provide administrative and clerical support to the Compliance Department and Chief Administrative Officer, manage compliance files and policies, assist with audits, contracts, incident tracking, meeting coordination and minutes, provide confidential support on HIPAA, labor, and legal matters, and serve as backup for HQ administrative functions.

Oversee the PACE programs Quality Improvement program, ensuring CMS and DHCS regulatory compliance, developing QAPI initiatives and KPIs, conducting audits and RCAs, training staff, preparing quality reports, and coordinating interdisciplinary performance improvement efforts to promote participant safety and regulatory readiness.
