Manages the Community Liaison Unit program, including staff supervision, grants and budget oversight, patient advocacy, outreach, education, referrals, care coordination, data reporting, and quality improvement. Collaborates with clinical leaders, community agencies, and internal departments to improve patient care and address community health needs. Oversees hiring, training, staffing, program meetings, service protocols, and compliance while representing the program to external partners.

Leads social work services for a PACE program serving frail and elderly participants. Responsibilities include psychosocial assessments, interdisciplinary care planning, counseling, referrals, advocacy, care coordination, documentation, home and facility visits, discharge planning, and caregiver support. Supervises Geriatric Services Specialists, supports quality improvement and regulatory compliance, and promotes culturally responsive care. Requires an MSW, healthcare and elderly-population experience, leadership skills, and Cantonese and/or Mandarin bilingual ability.

Leads the Foundation’s fundraising program, including major gifts, individual giving, events, campaigns, corporate engagement, donor operations, and development strategy. Manages donor portfolios, cultivates and solicits significant gifts, oversees fundraising systems and budgets, engages executives and board members, and supervises development staff. Builds donor relationships, strengthens the fundraising pipeline, monitors performance, and represents the organization with donors, community leaders, and philanthropic partners.
