Provides IPS-supported employment services to ACT participants by helping clients find, obtain, and retain work aligned with their goals. Builds collaborative relationships, assesses vocational needs, develops employment plans, supports benefits decisions, conducts employer outreach, and provides field-based services, including client transportation. Maintains records, follows IPS and ACT fidelity standards, attends team meetings, and participates in supervision and professional development.

Provide recovery-oriented support and intensive case management to individuals transitioning from Oregon State Hospital to community care. Coordinate with treatment teams, families, caregivers, and providers; assist with housing, benefits, employment, and services; develop crisis plans and treatment goals; document services; track outcomes; and use motivational interviewing and strength-based interventions within a multidisciplinary team.

Provides intensive, recovery-oriented case management and community navigation for individuals with significant mental health issues transitioning from Oregon State Hospital and other higher levels of care. Coordinates treatment teams, discharge planning, benefits, housing, employment, family and provider supports, crisis planning, assessments, documentation, and community services. Uses motivational interviewing, strength-based interventions, multidisciplinary collaboration, and electronic medical records.

Provides peer support to individuals with mental health conditions in community, home, clinic, and group settings. Helps clients create wellness plans, build independent-living skills, access community resources, attend appointments, and develop coping and communication strategies. Documents services, maintains professional boundaries, follows HIPAA and mandatory reporting requirements, supports crisis prevention, and coordinates with treatment teams in trauma-informed settings.

Provides case management, crisis intervention, individual and group skills training, mentoring, and community support for individuals with mental illness. Coordinates treatment plans with multidisciplinary teams, families, providers, hospitals, law enforcement, and community partners. Responds to crisis calls, develops safety plans, transports clients to services, maintains required documentation, and supports client re-entry into the community.

Provides peer support to individuals with mental health experiences through wellness planning, goal setting, coping strategies, skill building, crisis prevention, and community resource connections. Builds trusting relationships in community, home, clinic, and group settings; documents services; maintains professional boundaries; follows HIPAA, mandated reporting, trauma-informed care, and emergency response procedures.

Coordinates patient referrals by verifying insurance, obtaining authorizations and medical records, tracking referral status, communicating with patients, physicians, specialists, and internal teams, and resolving insurance or benefit conflicts. Documents referral activities, follows HIPAA requirements, supports prior authorization duties, and promotes patient safety and timely care coordination.

Supervises special mental health programs, staff, service delivery, compliance, documentation, audits, budgets, grants, and community coordination. Supports program development and the Program Director while ensuring adherence to state and federal regulations. Provides direct counseling, assessments, treatment planning, referrals, and crisis intervention as needed. Requires a master’s degree, five years of community mental health experience, supervisory or program management experience, and a valid driver’s license.

Manage accounting, financial tracking, reporting, audits, budgets, revenue contracts, invoicing, and receivables for housing, facilities, and construction projects. Coordinate LIHTC reporting and audit preparation, maintain facility accounting records, review project bids, and collaborate with housing, property management, facilities, project management, and finance teams. May assist with payroll and other accounting duties.
