Teams at Adapt Integrated Health Care

Provides IPS-supported employment services to ACT participants by helping clients find, obtain, and retain work aligned with their goals. Builds collaborative relationships, assesses vocational needs, develops employment plans, supports benefits decisions, conducts employer outreach, and provides field-based services, including client transportation. Maintains records, follows IPS and ACT fidelity standards, attends team meetings, and participates in supervision and professional development.

Provide recovery-oriented support and intensive case management to individuals transitioning from Oregon State Hospital to community care. Coordinate with treatment teams, families, caregivers, and providers; assist with housing, benefits, employment, and services; develop crisis plans and treatment goals; document services; track outcomes; and use motivational interviewing and strength-based interventions within a multidisciplinary team.

Provides intensive, recovery-oriented case management and community navigation for individuals with significant mental health issues transitioning from Oregon State Hospital and other higher levels of care. Coordinates treatment teams, discharge planning, benefits, housing, employment, family and provider supports, crisis planning, assessments, documentation, and community services. Uses motivational interviewing, strength-based interventions, multidisciplinary collaboration, and electronic medical records.
