Collaborate with clinicians and stakeholders to improve territory management, staffing levels, and operational efficiency.

Optimize workforce territories and staffing for more than 350 clinicians across the Bay Area and Sacramento. Responsibilities include assigning and balancing territories, assessing staffing levels, analyzing logistics data, identifying inefficiencies, generating performance reports, addressing operational challenges, and collaborating with clinicians and stakeholders to improve coverage and support staffing.

Supervise RN case managers and LVNs providing home-based patient care. Coordinate clinical escalations, case conferences, care-plan reviews, staff education, performance development, and quality improvement. Conduct supervisory field visits, review patient diagnoses and treatment, support therapeutic goal setting, and maintain customer relationships through outcome and readmission updates. The role combines three days of fieldwork with two days of San Jose office-based administrative and team-management duties.
