The primary purpose of this position is to manage care of one nursing unit in the Skilled Nursing Facility - up to 30 residents - within regulatory guidelines, and to support nursing management for the Assisted Living Facility.

 

ESSENTIAL DUTIES & RESPONSIBILITIES

  1. Manage resident care at the unit, ensuring communication, care coordination, and setting team direction. Monitor and manage acute changes in condition.
  2. Assist with Care Plans and family communication, as needed.
  3. Assist at the unit with admissions, discharges, and transfers, when applicable.
  4. Ensure compliance with current applicable federal, state and local regulations and facility policies and procedures. 
  5. Manage reports for Infection Control, Medication Errors, Pharmacy Recommendations, and other reports as directed by the Director of Nursing.
  6. Work with the IDT to identify and implement risk management programs, as applicable, for residents.
  7. Review documentation including progress notes, new orders, and assessments to ensure timely and accurate completion of health records. 
  8. Support the Infection Preventionist in training staff on infection control practices such as standard precautions, PPE usage, hand hygiene and monitoring compliance.
  9. Assist the Staff Development Coordinator with in-services, trainings, and competency evaluations, as needed.
  10. Identify any procedural areas of improvement and collaborate with Director of Nursing to resolve issues.
  11. Participate in quality improvement initiatives, audit programs, and root cause analyses as needed.
  12. Participate in nursing on-call rotation.
  13. Perform any other related duties as assigned.
Qualifications

Skills, Abilities & Competencies:

QUALIFICATIONS:

  1. Must possess a valid and current R.N. or L.P.N License in the State of Florida.
  2. Must be able to write, read, and speak the English language.
  3. Must possess and maintain a valid and current CPR certification.

 

PHYSICAL REQUIREMENTS:

1. Must be able to move intermittently throughout the workday.

2. Must be able to speak the English language is an understandable manner.

3. Must be able to cope with the mental and emotional stress of the position.

4. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position are fully met.

5. Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.

6. Must be in good general health and demonstrate emotional stability.

7. Must be able to relate to work with the ill, disabled, elderly, emotional upset, and at times, hostile people within the facility.

8. Must be able to lift a minimum of 50 pounds.

9. Must be free of communicable diseases.

 

EXPERIENCE:

1. Must have, as a minimum, one (1) years’ experience in Skilled Nursing Facility/Long-term Care Facility, or Assisted Living Facility.

2. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.

3. Must possess leadership and supervisory ability.

Skills Required

  • Valid and current R.N. or L.P.N. license in the State of Florida
  • Minimum one (1) year experience in a Skilled Nursing Facility/Long-term Care Facility or Assisted Living Facility
  • Knowledgeable of nursing and medical practices, procedures, and long-term care laws/regulations
  • Leadership and supervisory ability
  • Ability to read, write, and speak English
  • Valid and current CPR certification
  • Ability to lift a minimum of 50 pounds and move intermittently throughout the workday
  • Good general health, emotional stability, and ability to cope with job stress
  • Free of communicable diseases
