The REMOTE Transplant Waitlist Supporter will be responsible for the coordination of care for DaVita patients on a kidney transplant waitlist to help maintain their active status and help reactivate patients from inactive status. This coordination will be done through collaboration with local transplant partners. The Transplant Waitlist Supporter will provide support to patients while on the transplant waitlist by providing individualized education, support, and coordination of services.

The Transplant Waitlist Supporter is responsible for coordinating patient care across multiple areas – the patient and their care partners, living donors, DaVita clinics and local transplant partners.

ESSENTIAL FUNCTION

1. Coordinates on-going care needs of patients to remain active on the waitlist or to help reactive from inactive, including but not limited to medical, financial, psychosocial, nutrition, and pharmaceutical.

2. Educate on living donation and support patients on the waitlist to find a living donor

3. Serves as a liaison to DaVita clinics and local transplant partners in communication of patient needs.

4. Assess, plan, and advocate for the patient and their care partners in coordination of care along the continuum with DaVita clinics, local transplant partners, and other relevant parties.

5.Provides telephone consultation and follow-up to support patients and their care partners.

6. Remains knowledgeable of all listed patients' status on the UNOS waiting list to facilitate kidney placement in collaboration with local transplant partners.

7. Assists in the collection and analysis of data as requested by DaVita, patients and local transplant partners.

8.Knowledgeable of regulations related to transplantation and living kidney donation and follows guidelines to ensure compliance.

Salary/ Wage Range

$23.00 - $36.30 / hour

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
