M-F 8:30-5:00 w/ on-call every 4 weeks

Qualifications and Requirements:
- Field new patient/client inquiries over the phone in a knowledgeable manner
- Coordinate home health aides' schedules on a daily basis
- Monitor, mediate, and log both patient/client and home health aides in the system
- Follow up on home health assignments and patient/client services
- Participate in on-call rotation-serving as support for home health aides and residents

$20-24/hour
Skills Required
- Field new patient/client inquiries over the phone in a knowledgeable manner
- Coordinate home health aides' schedules on a daily basis
- Monitor, mediate, and log patient/client and home health aide interactions in the system
- Follow up on home health assignments and patient/client services
- Participate in on-call rotation to support home health aides and residents
- Work full-time in office at 160 Cleveland Street, Elyria, OH with M-F 8:30-5:00 schedule
