Position Summary:

Join Neighborhood PACE! As part of a dynamic Interdisciplinary Team (IDT) in an innovative model of care, you will work closely with participants and their caregivers to help them continue to live safely in their homes and communities as independently as possible. You will work in an integrated primary care and behavioral health setting to support participants in their homes and at one of our PACE centers. Full-time position to act as case liaison and support for the Neighborhood PACE Social Work department. Work with older adult participants and families to complete case management and housing tasks to maximize participant and caregiver satisfaction. An ideal candidate will be knowledgeable about community resources, be detail oriented, able to multitask, and possess excellent communication and computer skills. Must hold a Bachelor’s degree in social work or related field or have two years of relevant work experience in a community setting. Bilingual (English/Spanish) preferred. Automobile required.

The primary responsibility of the PACE Participant Navigator is to act as case liaison for PACE Social Work case management issues and problems to maximize participant and caregiver satisfaction.  The navigator may provide participants with informal supports and interventions under the direction of the social worker.

EDUCATION: BS or BA in social work or equivalent work experience in a community setting

EXPERIENCE: One year work experience with the elderly preferably in an interdisciplinary team setting

SKILLS/ABILITIES: Bilingual in English and Spanish:  Flexibility, creativity and good customer service skills. The ability and willingness to assist outside of traditional roles and with many disciplines.   Good computer skills.  Requires travel between the PACE centers.
