Supports social services in a long-term care and rehabilitation nursing center. Responsibilities include welcoming and admitting residents, completing psychosocial assessment, care plan, and discharge plan documentation, coordinating with the Social Services Manager, and promoting positive patient experiences during treatment. The role is Monday through Friday on a day shift.

As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center.  You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans.  You'll enjoy a Monday – Friday, dayshift work schedule.

Qualifications:

  • Minimum high school diploma or equivalent required.
  • Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
  • One year supervised social work experience working directly with individuals in a health care setting.
  • Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
  • One year supervised social work experience working directly with individuals in a health care setting.
  • One or more years’ experience in health care education/ marketing / insurance or commensurate educations preferred.
  • Must be familiar with medical terminology.
  • Proficient in the use of personal computer.

Pay Range

$18 - $18.50

Skills Required

  • High school diploma or equivalent
  • Bachelor's degree in social work or a human services field such as sociology, gerontology, special education, rehabilitation counseling, or psychology
  • One year of supervised social work experience working directly with individuals in a healthcare setting
  • One or more years of experience in healthcare education, marketing, insurance, or equivalent education
  • Familiarity with medical terminology
  • Proficiency using a personal computer
