As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. You'll enjoy a Monday -- Friday, dayshift work schedule.

Qualifications:
- Minimum high school diploma or equivalent required.
- Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and
- One year supervised social work experience working directly with individuals in a health care setting.

Pay Range
$20.10-$22.10
Skills Required
