Lead a nursing unit, support resident patient health, coordinate care, communicate with residents and families, and address concerns. The role requires an active RN or LPN license, an associate degree or higher in nursing or a practical nursing diploma, and at least one year of nursing experience. Prior sub-acute or long-term care experience is preferred.

Lead and mentor evening-shift nursing staff, support patient care, and help manage unit operations. The role works Monday through Friday from 3 PM to 11 PM and requires an active RN license and nursing experience. Prior sub-acute or long-term care experience is preferred.

Coordinate and implement the Resident Assessment Instrument and Minimum Data Set processes in a long-term care center. Ensure compliance with professional standards, physician orders, company policies, and local, state, and federal regulations. The role requires an active RN license and knowledge of Medicare, Medicaid, long-term-care payer systems, OBRA, RAI, and MDS processes, or prior long-term-care nursing experience.

Manage front desk operations at a long-term care/skilled nursing center: greet and check in visitors, direct calls, maintain directories and office records, assist applicants, and provide excellent customer service while multitasking between phones, email, systems and walk-ins.
