The Schools Team Coordinator is responsible for delivering prevention and early intervention services within partnering school districts. This role includes coordinating district-based strategic plans, providing educational programming and consultation, facilitating referrals, supporting students through classroom presentations, group education, and individual skill-building services, and maintaining documentation and outcome reporting required by funding sources. The position collaborates closely with school personnel, community partners, and agency leadership to ensure the delivery of high-quality services that promote student wellness and reduce high-risk behaviors.

Key Responsibilities:

· Develop and implement annual strategic plans with partnering school districts to address identified prevention and intervention needs.

· Deliver engaging classroom presentations, small group education sessions, and individual skill-building services that promote healthy decision-making, emotional wellness, social skills, and resilience.

· Collaborate with school personnel to identify students displaying at-risk behaviors and facilitate appropriate interventions and referrals.

· Provide training, consultation, education, and support related to Team of Concern universal and targeted interventions.

· Coordinate and support the on-site delivery of school-based prevention and early intervention services.

· Build and maintain positive relationships with students, families, school staff, community partners, and agency stakeholders.

· Complete assessments, consent forms, intakes, service documentation, and outcome reporting in accordance with agency and funding requirements.

· Protect student and family confidentiality while adhering to agency policies and procedures.

· Participate in clinical staffing, team meetings, quality improvement initiatives, and implementation of program recommendations.

· Provide outcome data and program documentation to leadership, grant management staff, funding sources, and community partners as required.

· Represent the organization through community presentations, outreach activities, networking opportunities, and special events.

· Promote effective communication and collaboration between school districts and agency programs.

· Support organizational goals and initiatives while performing other duties as assigned.

Education, Experience, and/or Credential Qualifications:

· Master's Degree from an accredited college or university in Social Work, Counseling, Criminal Justice, Psychology, Human Services, or a related field;

OR

· Bachelor's Degree from an accredited college or university in Social Work, Counseling, Public Health, Criminal Justice, Psychology, Human Services, or a related field and at least one (1) year of professional experience in a social service agency.

· Minimum of two (2) years of experience providing alcohol, tobacco, and other drug prevention programming for youth.

· At least one (1) year of experience serving as a site coordinator or in a position with primary responsibility for day-to-day prevention program implementation. This experience may be included within the required two years of prevention programming experience.

· Knowledge of prevention, early intervention, and treatment approaches for high-risk behaviors and substance use disorders.

· Knowledge of classroom management techniques and de-escalation strategies.

· Proficiency with electronic medical records and documentation systems.

Additional Qualifications:

· Current driver's license, acceptable driving record, and current auto insurance required.

· Ability to build rapport and maintain effective working relationships through strong communication skills.

· Ability to independently manage responsibilities while collaborating effectively with supervisors and team members.

· Ability to utilize established curriculum to develop and deliver effective prevention programming related to at-risk behaviors.

· Successful completion of background screening, including criminal record, driving record, abuse/neglect, and fingerprint checks.

Physical Requirements

· Perform sedentary work involving exertion of up to 10 pounds of force occasionally and a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects.

· Perform repetitive hand, finger, and arm movements for typing, writing, and computer-based work throughout the workday.

· Remain seated for extended periods while completing administrative and documentation responsibilities.

· Occasionally stand and walk as needed to carry out job duties within school and community settings.

· Ability to complete all essential job functions consistent with the physical demands of a sedentary work environment.
