Job Title: Scheduling Coordinator – Home Health

Position Overview:

We are seeking a highly organized and self motivated Scheduling Coordinator to support the daily operations of our home health and hospice agency. This role is essential in ensuring smooth administrative functions, efficient coordination of patient services, and seamless communication among staff, patients, and caregivers. The ideal candidate will thrive in a fast-paced healthcare environment, demonstrating exceptional multitasking skills, attention to detail, and a commitment to supporting quality patient care.

Key Responsibilities:

  • Office Operations: Manage day-to-day administrative tasks, including maintaining patient records, handling correspondence, and ensuring compliance with healthcare regulations.
  • Scheduling & Coordination: Assist with scheduling home health visits, ensuring proper documentation, and coordinating with clinical teams to optimize patient care services.
  • Communication Hub: Serve as the first point of contact for staff, patients, and families, providing professional and compassionate communication.
  • Compliance & Documentation: Maintain accurate and up-to-date patient records, process paperwork for new admissions, and ensure all documentation meets state and federal healthcare requirements.
  • Operational Support: Work closely with clinical staff, field nurses, and leadership to support office workflows and improve efficiency.
  • Inventory & Supply Management: Monitor and order medical and office supplies to support home health and hospice operations.

Qualifications:

  • Experience in an administrative or office coordination role, preferably in healthcare, home health, or hospice.
  • Strong organizational and time-management skills with the ability to handle multiple priorities.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and electronic medical records (EMR) systems.
  • Ability to work independently, anticipate needs, and take initiative in a fast-paced environment.
  • Excellent verbal and written communication skills with a compassionate and professional demeanor.
