Position Overview

The RN Clinical Supervisor is an experienced nurse responsible for ensuring that patient care and services are coordinated, managed and delivered appropriately. The RN Clinical Supervisor collaborates with other clinical leaders in directing care teams to provide the best possible care for all hospices in accordance to their unique Plan of Care.


Essential Functions and Skills

  • Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with the Medical Director or Hospice Physician regarding any questions about an individual's eligibility for services.
  • Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality hospice care services.
  • Available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plans of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Conducts quarterly record reviews and communicates findings and recommendations to the Manager of Clinical Services and hospice personnel.
  • Assists in the orientation of new employees.
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • Assists Manager of Clinical Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in implementing organization policies, procedures, goals and objectives, both short and long range.
  • Complies with accepted professional standards and principles.
  • Assesses the health needs of the community and assists in the development of plan(s)/ program(s) to meet needs. Participates in public relations and community activities that promote the organization's role as an effective member of the hospice and health care delivery system.
  • Promotes customer service orientation to all hospice personnel.

Additional Duties

  • Participate in on-call duties as defined by the on-call policy.
  • Attend interdisciplinary group meetings.
  • Assumes responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in- service classes.
  • Actively participates in quality assessment and performance improvement teams and activities as assigned.
  • Other duties as assigned.
Qualifications

Requirements/Qualifications

  • Must have a current Registered Nurse (RN) license in state(s) of practice.
  • Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.
  • Graduate of an accredited school of nursing required. Associate degree required.
  • Proven ability to work within an interdisciplinary setting required.
  • Ability to pass background, consumer report, and drug test.
  • If Driving: Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Preferred

  • Bachelor’s degree preferred.
  • Previous experience in hospice/homecare setting with 2 years management or supervisory experience strongly preferred.

  • Pushing/Pulling and Lifting/carrying up to 30 pounds. Occasionally up to 50 pounds.
  • Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.

Skills Required

  • Current Registered Nurse (RN) license in state(s) of practice
  • Graduate of an accredited school of nursing; Associate degree required
  • Proven ability to work within an interdisciplinary setting
  • Ability to pass background, consumer report, and drug test
  • If driving: Valid state driver's license, reliable automobile, required insurance, acceptable driving record
  • Bachelor's degree
