Registered Nurse RN Case Manager, Home Health
$5,000 Sign-On Bonus
Youngstown, Warren, Cortland, Poland and Boardman, Ohio

Elara Caring is hiring an RN Case Manager to provide home health care throughout Trumbull and Mahoning counties.

What You’ll Do
- Assess patients and develop individualized care plans.
- Provide skilled nursing care in patients’ homes.
- Teach patients and caregivers about medications, treatments, and managing their health.
- Coordinate care with physicians and the interdisciplinary team.
- Complete timely, accurate clinical documentation.
What You’ll Need
- Current, unrestricted state RN licensee
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Must be able and willing to travel 50%
- Associates Degree or Bachelor’s Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
