As a Registered Dietitian, you will play a critical role in supporting patients in the home health setting by assessing nutritional needs, developing individualized nutrition care plans, and providing education that promotes positive health outcomes. Working as part of an interdisciplinary care team, you will guide patients and caregivers in making informed dietary decisions, adapting recommendations to each individual's lifestyle, medical condition, and treatment goals. This role combines clinical expertise, patient education, and collaborative care coordination to help improve overall health and quality of life.
Primary Responsibilities:
  • Conduct comprehensive nutritional assessments and develop individualized nutrition care plans that are incorporated into the patient's plan of care
  • Monitor, evaluate, and document patient progress, outcomes, and recommendations
  • Provide nutrition counseling and education to patients, families, and caregivers in the home setting
  • Educate individuals on dietary principles, meal planning, food selection, and cost-conscious nutrition strategies
  • Adapt nutritional recommendations to accommodate patient preferences, lifestyle needs, and medical conditions
  • Reassess and modify nutrition care plans as needed through ongoing follow-up visits
  • Maintain accurate, timely, and complete clinical documentation within the medical record, including assessments, care plans, progress notes, and discharge summaries
  • Collaborate with the interdisciplinary care team to communicate patient progress and support treatment goals
  • Protect patient confidentiality and ensure compliance with all regulatory, organizational, and professional standards
  • Follow agency policies, procedures, and clinical practice guidelines
  • Perform additional duties as assigned

Required Qualifications:
  • Bachelor's or Master's degree from an accredited college or university
  • Active and unrestricted Registered Dietitian license in the State of Nevada
  • Current CPR certification, or ability to obtain upon hire
  • 1+ years of experience as a Registered Dietitian
  • Experience working with electronic medical record (EMR) systems
  • Basic computer proficiency, including Microsoft Windows, Outlook, and email applications
  • Demonstrated knowledge of current dietetic standards of practice and applicable state and federal regulations
  • Demonstrated solid verbal, written, organizational, and patient education skills
  • Demonstrated ability to effectively prioritize tasks and communicate clinical objectives
  • Valid Nevada driver's license, reliable transportation, and personal auto insurance that meets organizational requirements

Preferred Qualifications:
  • Bachelor's or Master's degree in Nutrition, Dietetics, or a closely related field
  • Home health, community health, or post-acute care experience
  • Experience working with diverse patient populations and chronic disease management
  • Experience collaborating within interdisciplinary healthcare teams
  • Bilingual language skills

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $49,700 - $88,800 annually based on full-time employment. We comply with all minimum wage laws as applicable.
